What is PCCM’s Health Concierge Services?

  • Assistance finding and connecting to physicians and other medical providers who accept your insurance or payment method, have availability and are well rated.
  • Assistance completing medical questionnaires, reviewing insurance EOB, and medical provider charges.
  • Someone to schedule physician visits and procedures and remind you of upcoming visits.
  • Someone to review and reconcile prescription medications.
  • Assistance returning home from a hospitalization or skilled nursing facility stay.
  • Assistance arranging travel logistics for a family member with medical complications- like help finding a doctor, home health company and arranging transportation assistance.
  • Help finding, evaluating and choosing caregivers.
  • Explanation of and assistance accessing different types of community services. (Home Health, Hospice, In-home and Consumer Directed Services, Transportation, etc.)
  • Routine check-up calls from a nurse.
  • Chronic disease and disease process education.
  • Someone to help develop diet and exercise programs and to provide motivation to stay with them.
  • 24X7X365 Nurse Helpline.
  • Home Nurse visits. (May also accompany you to physician visits)
  • Value added pharmacy services like free multi-dose packaging, home delivery and reminder calls.
  • Someone to train family and paid caregivers on care techniques and equipment.
  • Assistance finding better prescription prices, discounts and rebates.
  • Additional Service Option: Epharmix’s disease specific phone and text messaging. Additional fees apply.
  • Additional Service Option: Honeywell’s Telemonitoring and Telehealth. Additional fees apply.

How do we Charge for our Service?

3 Payment Plans to Choose from – Included in each:
PCCM BRONZE PLAN$20.00 on the 1st of each month/No sign-up fee
  • Up to 20 minutes of direct concierge services monthly/240 minutes annually.
  • Individual Beneficiary is eligible.
  • Free medication synchronization, multi-dose packaging, home delivery of prescriptions through PCCM affiliated Pharmacy.
PCCM SILVER PLAN$52.00 on the 1st of each month/$49.00 sign-up fee
  • Up to 40 minutes of direct concierge services monthly/480 minutes annually.
  • Enrolled Member + up to one additional household resident is eligible.
  • Includes 2 annual nursing visits.
  • Option to add quarterly nursing visits for $6 per month per added visit.
  • Option to roll-over unused quarterly nursing visits to following quarters or convert to Health Concierge services.
  • Option to purchase Personal Care, Household Assistance and Respite Services for $19 per hour with no-minimum purchase requirements.
  • Free medication synchronization, multi-dose packaging, home delivery of prescriptions through PCCM affiliated Pharmacy.
PCCM GOLD PLAN$84.00 on the 1st of each month/$49.00 sign-up fee
  • Up to 60 minutes of direct Health Concierge services monthly/720 minutes annually.
  • Enrolled Member + up to two additional household residents are eligible.
  • Includes 4 annual nursing visits.
  • Option to add quarterly nursing visits for $6 per month per added visit.
  • Option to convert unused Health Concierge service balances to nursing visits, personal care, homemaker, or respite services.
  • Option to roll-over unused quarterly nursing visits to following quarters or convert to Health Concierge services.
  • Option to purchase Personal Care, Household Assistance and Respite Services for $19 per hour with no-minimum purchase requirements.
  • Free medication synchronization, multi-dose packaging, home delivery of prescriptions through PCCM affiliated pharmacy.
EPHARMIX$13.50 on the 1st of each month/$13.50 sign-up fee
  • Disease Specific Phone and text messaging
  • Epharmix delivers automated phone call and text messages to collect and analyze disease-specific data and send actionable, real time alerts to the patient’s PCCM care coordinator. PCCM will triage all alerts and take necessary and progressive interventions where a change in condition, or potential risk eminently exists. Call and Text interactions may be customized to address the risks associated with multiple chronic conditions and include clinically validated protocol for Heart Failure, COPD, Diabetes, Hypertension, Pneumonia, Dialysis, Fall Prevention and Wounds among others.
HONEYWELL GENESIS 3.2 $169 on the 1st of each month/$169 sign-up fee
  • Tele-Montioring/Tele-Health
  • Honeywell Genesis 3.2 Tele-Monitoring/Tele-Health is delivered via a Samsung Galaxy Tablet 8.0 enabled with Android OS, Wi-Fi, and 4G/3G connectivity to ensure connection in even the most remote locations. The system provides for symptom specific assessment and monitoring of hypertension, COPD, CHF and diabetes with peripheral device connectivity for digital scale, pulse oximeter, glucose meter, peak flow meter, PT/INR meters, blood pressure cuff, ECG. Integrated video permits patients to interact with Physicians, providers and families simultaneously with optional call recording and playback.
Terms & Conditions

Enrollment fee and 1st month’s payment due prior to membership activation.
Payment due monthly on the 1st of the month thereafter.
All credit balances must be used within 12- months of initial enrollment or membership anniversary date thereafter or they are forfeited without refund.
Nursing visits included in the gold and silver packages may only be used at a rate of one visit per quarter.
Nursing visits added to the gold or silver package by adding $6 to the monthly payment amount per visit may only be added at a rate of 1 visit per quarter. (For example, if a member chooses to add two additional nursing visits to the gold package by adding $12 monthly to the member fee, the member will be eligible to use up to 2 nursing visits in the 1st quarter following enrollment and may add, at the member’s preference, the additional nursing visit in any of the following quarters)
Minimum 24-hour advanced scheduling required for all nursing visits.
Gold plan member’s convertible credit rate of unused concierge services into nursing, personal care, household assistance or respite services is $.74 per minute.
Gold and silver plan member’s convertible credit rate of unused nursing visits into concierge services is at a rate of 1 visit for 67 minutes of concierge services.
PCCM reserves the right to use LPN or RN to complete nursing visits at its discretion and per compliance with Missouri laws and regulations.
Prices are subject to change after 12-months of enrollment and annually thereafter.
Membership may be terminated with or without cause by member or PCCM with 24 hours advanced notice.
